Join Agape Care Group as a Hospice Liaison, where you will play a pivotal role in connecting our compassionate care with patients and families during profound moments of their lives. As part of our dedicated team, you'll focus on enhancing our outreach and ensuring a seamless transition for patients entering hospice care. Your expert communication and deep understanding of patient needs will bridge the gap between medical providers, patients, and our hospice services, aligning with our mission to deliver dignified and personalized end-of-life care. Coordinate and manage patient referrals from healthcare providers and facilitate the effective transition into hospice care. Educate families and patients about hospice services and the personalized care plans provided by Agape Care Group. Build and maintain strong relationships with healthcare providers, community organizations, and internal teams to ensure coordinated and comprehensive patient care. Attend and participate in community events to raise awareness about hospice care benefits and services. Regularly update patient records and use CRM systems to monitor and report on referral activities and outcomes. Excellent interpersonal and communication skills Strong organizational and multitasking abilities Experience with CRM systems and patient management software Ability to work compassionately with patients and families facing end-of-life decisions Knowledge of the healthcare system and patient care processes Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid driver’s license and reliable transportation Preferred: Certified Hospice and Palliative Liaison (CHPL)
