Job Description:
Job DescriptionThe incumbent will lead the day-to-day local
operations of the Hospice's Satellite office. This individual will
assist in planning, implementation, delivery and supervision of
direct clinical services provided to patients and families. Assists
with visits, call and other nursing duties as needed. May act as RN
Case Manager.QualificationsGraduate of an accredited school of
nursing. One to two years of recent acute care experience in an
institutional setting, and one to two years of recent experience in
home care preferred. Minimum two years nursing experience
required.Current licensure in State, CPR certification and valid
drivers license.Bachelor's degree, with one year of hospice care
experience preferred.Demonstrate ability to manage all aspects of
terminal care with expertise in pain and symptom management and
hospice nursing.Demonstrate skill in administrative performance,
including expertise in both written and oral communication,
supervision of staff, documentation requirements, service
coordination and case management.Demonstrate ability to execute an
effective, supportive, sensitive approach in response to needs of
staff and patient/family.Excellent observation, verbal and written
communication skills, problem solving skills, basic math skills;
nursing skills per the competency checklist.Prolonged or
considerable walking or standing. Able to lift 75 pounds, position
and/or transfer patients. Able to lift supplies and equipment.
Considerable reaching, stooping bending, kneeling and/or crouching.
Visual acuity and hearing to perform required nursing skills.Must
have reliable transportation for daily travel throughout the
service area.Flexibility to meet patient needs necessary. Some
evening, night, early morning, holiday and weekend hours required
during on-call hours, and during periods of high patient needs and
to ensure hospice services are provided 24 hours per day and 7 days
per week.Demonstrates ability to effectively learn and use computer
and required software programs to ensure a true and correct
electronic medical record for each patient.Must be able to utilize
technology appropriately as needed to meet the requirements of the
position
