Job Description:
Agape Care Group is seeking a compassionate and dedicated
Registered Nurse to provide hospice care during after-hours and
weekends, ensuring that patients in the final stages of life
receive empathetic and proficient medical support. As a key member
of our hospice team, you will work autonomously to manage patient
care in various settings, such as homes and nursing facilities,
upholding our commitment to enhancing the quality of life of both
patients and their families. This position plays a vital role in
carrying out Agape Care Group's mission by extending essential care
during potentially critical times. Provide direct patient care
according to the established patient plan of care. Coordinate with
physicians, healthcare providers, and families to implement
patient-centered care plans. Administer medications and monitor
patients' vital signs, reporting significant changes and
maintaining accurate records. Offer emotional and practical support
to patients and their families during end-of-life care. Educate
families and caregivers on patient care and management to ensure
continuity of care. Participate in interdisciplinary team meetings
to review and optimize patient care plans. Proficient in providing
end-of-life care and symptom management Ability to make independent
decisions when circumstances warrant Strong organizational and
communication skills Compassionate and emotionally resilient Adept
in handling personal, sensitive, and confidential situations

CPR certification Certified Hospice and Palliative Nurse (CHPN)
