Job Description:
Agape Care Group is seeking a compassionate and dedicated Nurse
Practitioner to join our Palliative Care team. In this role, you
will be an integral part of delivering personalized,
patient-centered care aimed at improving the quality of life for
patients facing serious illness. Our Nurse Practitioners provide
expert symptom management, advanced care planning, and emotional
and spiritual support tailored to the individual needs of each
patient. Join us in our mission to ensure peace and dignity during
lifes most meaningful moments. Evaluate patient conditions and
establish appropriate palliative care plans. Provide direct patient
care to palliative care patients and manage symptom relief.
Collaborate with an interdisciplinary team including physicians,
social workers, and chaplains to enhance the quality of life for
patients and their families. Educate patients and families about
the disease process, end of life care, and medical options.
Implement treatment plans while ensuring the consistency of care
across all team members. Participate in on-call rotation to provide
comprehensive coverage and support for urgent care needs. Excellent
patient management skills and empathy. Strong clinical skills with
an emphasis on palliative care and symptom management. Ability to
communicate effectively with patients, families, and
interdisciplinary teams. High level of organization and the ability
to handle multiple tasks simultaneously. Medical Insurance Dental
Insurance Vision Insurance 401k Current Nurse Practitioner license.
Certification in Palliative Care Nursing (CPN) preferred.
